To provide optometric care we process, among other things:
Name, address and town
Date of birth
Phone number and email address
Citizen service number (BSN) (where legally required for healthcare claims)
Details of your GP or medical specialist (if applicable)
Health insurance details
Medical data from optometric examinations
Notes on your health and medical history
We process your data to:
carry out optometric examinations and eye care
assess and monitor your eye health
schedule and confirm appointments
submit claims to health insurers
meet legal obligations, such as record keeping and retention periods
We process your data on the basis of:
consent when you share data with us voluntarily
a treatment agreement for providing care
legal obligations, such as the Dutch WGBO and Wkkgz acts
legitimate interest, for example to improve the quality of our services
Your data is stored in a secure patient record. Only authorised staff have access to your data.
We use secure systems and connections to protect your data.
By law, medical records are kept for at least 20 years.
We only share your data when necessary for providing care, for example:
with your GP or medical specialist (with your consent)
with health insurers for claims
with IT suppliers for secure storage of patient data
With these parties we always conclude data processing agreements .
You have the right to:

have your data corrected
object to processing
request transfer of your data
file a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens)
